○The Social Services Practitioner will reassess the child's Concurrent Plan for legal permanence at the next review for possible placement in a relative or licensed caretaker's home who will commit to adoption or legal guardianship should efforts to reunify with parent or parents fail.
○The Social Services Practitioner will assess relatives for possible placement who are identified and may come forward and indicate an interest in providing a permanent home for the children should reunification efforts with the parent fail. And/or;
○The Social Services Practitioner will seek a licensed foster family home committed to adoption or legal guardianship of the children should efforts to reunify fail. Upon locating such a home, the Social Services Practitioner may place the child in this home.
